Affiliated Instructors: Record Submission

Immediately following your class, be sure that you have completed your Course Roster file (click to download a blank copy). If a student did not pass the class or did not finish, do not include their information on the roster you upload.

Every student must have a First Name, Last Name, separate/unique Email Address, and a Phone Number. A unique email address belonging to the student is required for each person - a single company email can’t be used for multiple students.

Once you have a filled Excel Roster submit it using the form below. After submission, you will be taken to the payment page. Payment must be made in full in order for Ohio CPR Certs to process you student certifications.
